CARE MBA provides microinsurance to a diverse membership base encompassing cooperative partners, formal organizations, DSWD members, church groups, and other institutions.
Simple requirements in Membership Application (Ages 18-65years old):
- Membership Application form (Application form#1 to be filled up by member)
- Birth certificate of the member
- Marriage Certificate (if married)
- Birth Certificate of children
- Marriage contract of parents below 65 years old (if single)
